Travel Licensed Rehabilitation Therapist - Skilled Nursing Facility

Location Hawaii Assignment Duration This is a 13-week contract position. Schedule Variable schedule - 5 days per week, including potentially one weekend day. Job Description The Licensed Rehabilitation Therapist will provide rehabilitation services in a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). Responsibilities include conducting evaluations, reassessments, and treatments, while working collaboratively with an interdisciplinary team to develop and implement individualized treatment plans. Responsibilities Provide clinical rehabilitation services according to licensure and scope of practice. Develop, implement, and carry out individualized treatment plans in collaboration with the supervising therapist and physician. Utilize evidence-based therapeutic interventions to maximize residents' function, independence, and quality of life. Educate residents, families, and caregivers on treatment plans and progress. Complete accurate and compliant documentation of patient care activities. Supervise therapy assistants as applicable. Be available to work on Hawaii State and Federal Holidays as necessary based on facility operations.

Required Experience / Certifications / Licensure A minimum of two (2) years of experience in a Skilled Nursing Facility or Intermediate Care Facility setting preferred. Current State of Hawaii license in good standing appropriate to the discipline. Current Basic Life Support (BLS) certification.
