- Bachelor's or Master's Degree in Social Work (BSW or MSW)
- Minimum Licensed Social Worker (LSW) certification required
- Case management experience
- Experience in a clinical or healthcare multidisciplinary team setting

- Monday-Friday, 8:00 AM - 4:00 PM (1-hour unpaid lunch)
