Licensed Practical Nurse

Ontario Center seeks a compassionate Licensed Practical Nurse to provide hands-on care in our skilled nursing and rehab facility. The LPN will deliver medications and treatments, monitor resident status, and support restorative therapies in a resident-centered environment. Responsibilities include assisting with ADLs, documenting care, collaborating with an interdisciplinary team, and promoting safety, dignity, and comfort. This role offers opportunities to grow clinical skills, participate in care planning, and positively impact residents’ health and quality of life through attentive, team-based nursing care.
