Care Coordinator

Hours: 40 hours per week. Work schedule is Monday, Tuesday, Thursday, and Friday, 8:30 am to 5:00 pm or 9:00 am to 5:30 pm; Wednesday, 10:30 am to 7:00 pm Duration: 3 to 4 months, depending on program needs As a Care Coordinator, you will: Work closely with various providers in the region or system to facilitate access to program services for those identified as the target population to be served.Conduct case management, linkages and referrals to needed supports in the community.Act as a liaison between the clinic and community partners to facilitate referrals and coordination of care for existing clients.Assist the client in meeting client service plan goals, prioritizing areas of need, and steps needed to reach goals established, as well as identified areas of strength.Engage and follow
