In this role, you will coordinate patient care, manage utilization reviews, and oversee discharge planning to ensure high-quality outcomes. You will act as the primary liaison between patients, families, healthcare providers, and insurance companies. Key Responsibilities Coordinate patient admissions , transfers, and discharge planning. Conduct utilization reviews to ensure appropriate level of care and insurance reimbursement. Collaborate with physicians , therapists, and nursing staff to develop individualized care plans. Communicate with insurance providers to obtain authorizations and manage concurrent reviews. Serve as an advocate for patients and families, guiding them through the healthcare continuum. Maintain accurate documentation in accordance with state, federal, and facility regulations. Qualifications Active LVN license in the State of California. Minimum 1-2 years of case management or utilization review experience (preferred). Experience in skilled nursing (SNF) or long-term care settings highly desired. Strong knowledge of Medicare, Medi-Cal, and managed care insurance processes. Excellent communication , organization, and multitasking skills. Proficiency with electronic health record (EHR) systems.
