What You'll Do

As a Registered Nurse, you'll play a vital role in helping patients recover, manage chronic conditions, and remain safely at home.

Responsibilities include:

  • Performing comprehensive nursing assessments in patients' homes.

  • Developing, implementing, and updating individualized plans of care.

  • Providing skilled nursing care, treatments, and patient education.

  • Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.

  • Monitoring patient progress and communicating changes in condition.

  • Educating patients and caregivers to promote confidence and independence.

  • Maintaining timely and accurate clinical documentation.

  • Delivering care in accordance with regulatory and organizational standards.


Qualifications Required
  • Current, unrestricted Texas Registered Nurse (RN) license

  • Minimum one (1) year of nursing experience

  • Strong assessment and critical thinking skills

  • Valid driver's license, reliable transportation, and current auto insurance

  • Ability to travel throughout the assigned service area

  • Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50100 pounds, including assisting with patient transfers and moving equipment.
Preferred
  • Home Health or Hospice experience

  • OASIS documentation experience

  • Homecare Homebase (HCHB) experience

  • Associate or Bachelor's Degree in Nursing
