Nurse Case Manager

TriStar Health seeks a Nurse Case Manager to coordinate patient-centered care across our regional network. In this role, you’ll assess clinical needs, develop individualized care plans, and guide patients through hospital stays and post-acute services. You will collaborate with physicians, bedside nurses, social workers, and community partners to support safe transitions, reduce readmissions, and improve outcomes. Drawing on evidence-based practice and TriStar’s patient-first culture, you’ll educate patients and families, ensure access to resources, and document care in the EHR while contributing to quality and performance improvement efforts.
