Case Manager Social Worker

LifeBridge Health seeks a Case Manager Social Worker to coordinate care for patients across a comprehensive continuum of services. In this role, you will complete psychosocial assessments, create and manage care and discharge plans, and connect patients and families with community and social service resources. You will work closely with physicians, nurses, and an interdisciplinary team to remove barriers to care, advocate for patients, and support safe, timely transitions.
