Job Description
Evaluates patients in their homes and provides occupational therapy to support rehabilitation, functional ability, and safety. Develops and documents individualized plans of care, tracks progress, recommends adaptive equipment and home modifications, and coordinates treatment with patients, caregivers, physicians, and the broader care team.
