GENERAL SUMMARY

Works under the supervision of the Administrative Director of Population Health. The Care Center Nurse Manager’s primary function is to oversee the operations of the Care Center and assist with organizational Population Health initiatives. This role is responsible for the enforcement and accountability of policies, procedures, projects, and tasks. Works in collaboration with physicians, APPs, SOMC leadership, social work, dieticians, pharmacy, etc to implement initiatives and assist with reaching Population Health goals. The manager is expected to create and sustain a high performing team.

QUALIFICATIONS

Education:

• High School Diploma or successful completion of an equivalent High School Exam Required

• Graduate of an accredited school of practical nursing required

Associates degree preferred

Licensure:

• Licensed to practice as an LPN in Ohio required

Experience:

  • 4+ years of experience of healthcare experience required
  • 2+ years of leadership experience preferred
  • Working knowledge of Microsoft Word, PowerPoint and Excel preferred
  • Experience with data visualization tools preferred

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS

The following is a summary of the major job duties of this job. Other duties may be performed, both major and minor, which are not mentioned below. Specific activities may change from time to time.

1. Oversees daily operations of the Care Center.

2. Coordinates department orientation and training for new hires.

3. Monitors employee hours and schedules in the payroll system to ensure proper use of PTO, overtime, etc.

4. Manages the Care Center personnel through daily huddles, rounding, continuous performance feedback, and annual reviews.

5. Interacts with SOMC leadership to discuss quality improvement projects, education on Care Center activities/tasks, and Population Health data and initiatives.

6. Performs audits when necessary to inspect what is expected, holding staff accountable to complete work.

7. Updates dashboard(s) monthly and maintains action plans for measures not meeting the goal

8. Assess patients over the phone and escalate to physicians and APPs as appropriate.

9. Assists with the coordination of care for the patient in collaboration with the Care Center staff and other SOMC leaders and staff to meet the needs of the patient.

10. Accurately and timely communicates with staff to share changes in workflows and procedures

11. Continues to develop leadership skills and competencies.

12. Works with Population Health Data Analyst to review payer and contract data to maintain accurate alignment with performance targets with Value Based Care.

13. Combines evaluations of data analytics, operations and care team workflows to ensure insights gained become embedded improvements in daily practice.

14. Collaborates across the organization and physician clinics to ensure efficiency, frontline workflow improvements and optimized solutions that are aligned with our Population Health goals

15. Performs other duties as assigned.
