Pay Range: $38/hr - $42/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
Conduct in-person member home visits for assessments and care coordinationReview medical records and prepare for visits to ensure comprehensive evaluationsSubmit assessments within 24 hours and complete documentation on timeCoordinate member care and facilitate transitions to permanent care managers/service coordinatorsComplete required training and assigned tasks promptly
Key Requirements and Technology Experience:
Active RN license in McAllen Area, TX3+ years of nursing experience2+ years of case management experienceExperience in pediatric care is preferredStrong clinical judgment, accountability, and prioritization skills
