Behavioral Health Specialist Fellow

Oak Street Health seeks a Behavioral Health Specialist Fellow to provide integrated behavioral health services within a primary care setting for adults on Medicare, many in underserved communities. In this fellowship role, you will conduct psychosocial assessments, brief evidence-based interventions, safety and risk assessments, and care planning in collaboration with physicians, nurses, and care teams. You will support care coordination, connect patients to community resources, and document in the EHR. Supervision and training are provided, with opportunities to grow clinical skills, pursue licensure, and contribute to a patient-centered, inclusive care model.
