Trellis Supportive Care is seeking compassionate PRN/Per Diem Hospice Admissions Nurses to provide exceptional in-home assessments and care for individuals considering or in need of hospice services.

As a Hospice Admissions Nurse, you will play an important role in helping patients and families understand and access hospice care.
Your responsibilities will include:
- Assessing and evaluating the needs of hospice patients and families in their homes
- Determining eligibility for hospice services prior to admission
- Educating families about hospice services and available support
- Establishing appropriate nursing care plans based on the needs of the patient and family
- Providing compassionate, exceptional patient care in accordance with the plan of care
- Communicating with physicians and other members of the interdisciplinary team to ensure high-quality care
- Building trusting relationships with patients and families through exceptional communication and support
- Helping patients and families navigate their hospice journey with compassion, dignity, and respect
- Current North Carolina Registered Nurse (RN) license
- Minimum of two (2) years of clinical experience preferred
- Excellent nursing assessment and problem-solving skills
- Strong interpersonal and communication skills
- Exceptional customer service skills
