The Medical Pod Leader MD/DO – Home-Based Primary Care serves as the clinical leader for a multidisciplinary care “pod,” supervising APPs and physicians while providing in-home primary care to medically complex patients. This role drives accurate diagnosis and documentation, value-based quality metrics, and patient safety, leads high-risk huddles and chart audits, mentors APPs, supports EMR efficiency, and partners with operational leaders to improve pod performance and patient outcomes.
