Hospice RN Case Manager

In this field-based Hospice RN Case Manager role, youll manage your own Victoria-area caseload, help patients find comfort sooner, and guide families through some of the most important moments of carewithout the constant chaos of a hospital floor. Opportunity

Youll join a growing hospice team serving patients across Victoria, TX and surrounding communities. This is a full-time, field-based role with clear ownership of your day, support from an interdisciplinary care team, and room to grow into leadership over time.

What You Will Own
  • Manage a hospice nursing caseload across Victoria-area communities.
  • Complete nursing assessments in patients homes and care settings.
  • Build and update individualized care plans with the physician, social worker, chaplain, CNA, and other care partners.
  • Teach patients and families about pain control, symptom management, medications, and what to expect during hospice care.
  • Coordinate care so patients receive comfort-focused support quickly and families feel prepared.
  • Keep EMR documentation accurate, complete, and on time.
  • Participate in a shared weekday and weekend on-call rotation with fair coverage.
Schedule, Location, and Work Model
  • Full-time, field-based role serving Victoria, TX and surrounding communities.
  • MondayFriday schedule, 8:30 AM5:30 PM.
  • Weekday on-call is approximately 2 times per week.
  • Weekend on-call is approximately 1 in 3 weekends.
  • Must live within reasonable distance of Victoria, TX to support field visits and on-call needs.
Compensation

The expected total compensation range is USD 75,00085,000 Yearly.

What Makes You a Strong Fit
  • Active Texas RN license or valid multistate RN compact privilege to practice in Texas.
  • Able to work field-based across Victoria-area communities.
  • Comfortable supporting patients and teaching families during difficult, emotional moments.
  • Able to manage documentation accurately in an EMR.
  • Ready to participate in both weekday and weekend on-call rotation.
  • Organized enough to manage a caseload while moving between homes and care settings.
Nice-to-Haves and Transferable Backgrounds

Hospice experience is helpful, but it is not the only path into this role. Strong candidates may also bring experience from:

  • Home health.
  • Skilled nursing facilities.
  • Long-term care.
  • Emergency room nursing.
  • ICU.
  • Step-down units.
  • Other settings where youve supported patients and families through complex clinical situations.

Bilingual skills are also a plus.
