Clinical Pharmacist Ambulatory Cardiology

Intermountain Health seeks a Clinical Pharmacist for our ambulatory cardiology clinics. In this role, you will provide direct patient care through medication therapy management, anticoagulation and heart-failure pharmacotherapy, and optimization of cardiovascular drug regimens. You will collaborate closely with cardiologists, APPs, nurses, and care managers, perform clinical consults, adjust therapy under collaborative practice agreements, monitor labs and outcomes, and educate patients and staff. The position supports quality initiatives, protocol development, and precepting learners in a mission-driven, team-based environment.
